Guide to filling repair request form
1. Name: Please provide your name (preferably the name stated in your invoice)
1. නම: කරුණාකර ඔබේ නම සපයන්න (වඩාත් සුදුසු වන්නේ ඔබේ
ඉන්වොයිසියේ සඳහන් කර ඇති නම)
2 and 3. Address: Please state the address and city of the site location
2 and 3. ලිපිනය: කරුණාකර ලිපිනය සහ නගරය සඳහන් කරන්න.
4 and 5. Email and Phone: Please provide your contact details
4 and 5. විද්‍යුත් තැපෑල සහ දුරකථනය: කරුණාකර ඔබේ සම්බන්ධතා තොරතුරු සපයන්න.
6. Select your service: Please select your installation
6. ඔබේ සේවාව තෝරන්න: කරුණාකර ඔබට අවශ්‍ය සේවාව තෝරන්න.
7. Comments: Please explain the malfunction/problem with your installation that needs attention. A better explanation such as specific details of the problem and when it started will help us provide you with a faster service.
Example: My swing gate does not operate. I have checked the power supply and all the breakers are intact. The problem started yesterday after a heavy storm.
7. අදහස්: කරුණාකර අවධානය යොමුකළ යුතු ඔබට අවශ්‍ය සේවාව / ගැටලුව පැහැදිලි කරන්න. ගැටලුවේ නිශ්චිත තොරතුරු සහ එය ආරම්භ වූ දිනය නිවැරදිව ලබාදීමෙන් ඔබට වේගවත් සේවාවක් ලබාදීමට එය අපට උපකාරී වේ.
උදාහරණය: මගේ පියන් දෙකේ ගේට්ටුව ක්‍රියාත්මක නොවේ. මම විදුලි සැපයුම පරීක්ෂා කර ඇති අතර සියලුම බ්‍රේකර් නොවෙනස්ව පවතී. මෙම ගැටළුවඊයේ ආරම්භ වූයේ දැඩි කුණාටුවකින් පසුවය.
